| 【 要旨 】 | 看護診断するために必要なアセスメント能力を高めることは，看護の質を向上させるための要因となる．近年，看護の質が問われているなか，行った看護の意味づけや看護の科学的裏づけとなるアセスメント能力を高めることは重要な課題である．また，アセスメントしたものを正しく記録に残し，実践の場に生かしていくためには，(1) 自己研鑚するとともに，チーム全体でアセスメント能力をレベルアップする，(2) 実践の場に活用しやすい記録様式の改善を行う，(3) 看護実践のなかで，タイムリーにアセスメントし記録する習慣をつける，(4) 看護診断したものを継続して実践の場に生かせる看護体制作りなどが必要であることが実践の場を通して明らかになった． それらの問題を解決するとともに，日々の看護実践のなかで，一つひとつの症例を大切にし，行った看護を評価し，学習を積み重ねることによりアセスメント能力をレベルアップし，看護の質の向上に結びつけることができる． |

| [ Summary ] | Our purpose was to improve assessments for nursing diagnosis, which define nursing practice and give a scientific basis to it. Since today nursing quality has become an important theme, the key to quality of care is doing so. The following are required, (1) improving the ability of the each staff member, (2) making a useful format, (3) getting in the habit of recording the assessment each time, (4) systematization for continuing. Nurses can improve the ability of assessment and the quality of care by paying attention to each case to make an assessment of step by step efforts during their daily practice. | |
