| 【 要旨 】 | 情報収集は，患者の問題を正しく判断し，ケアしていくためにもっとも大切な要素である．しかし，情報収集段階での看護婦の経験や能力差によって情報収集の視点が異なれば，患者の健康問題のとらえ方や解釈に偏りが生じてくる危険性がある．当院はゴードンの分類の「機能面から見た健康パターン」の11のアセスメントカテゴリーを使用しデータベースとしているが，一般的な情報収集の視点であるため，疾患や病期の違いなどにより生じる特殊性については，各部署で独自のアセスメントの視点をもつことが必要である． 当病棟では腎疾患患者のもつ社会的・身体的・精神的な特殊性に加え，教育・指導的な関わりを把握するうえで必要な情報収集に腎疾患アセスメントツールを作成し活用しているので紹介する． |

| [ Summary ] | Gathering information from patients is of basic importance in understanding and taking care of their problems. However if nurses, whose career and ability vary, ask questions from their individual point of view, it may lead incorrect interpretation of those health problems. For this reason our hospital has a standardized database, but it is composed of information from the larger,general point of view. It is still necessary to have an individual viewpoint on the division basis,taking into account the different character is tics and duration of diseases. In this study, considering the social, somatic, and mental characteristics of patients with kidney disease, we construct our own database, which comprises several important items for our educational program. | |
