【 要旨 】 近年，ホモシスチン尿症をはじめとした高ホモシステイン血症をきたす疾患・病態においては，遊離型，蛋白結合型を含めた総ホモシステイン(tHcy)を評価することが重要であるといわれている．tHcy測定法としては，radioenzymatic assay,gaschromatography-mass spectrometry，高速液体クロマトグラフィー(HPLC)法，免疫法など，さまざまなものが開発されてきた．われわれの施設では，カラム前誘導体化HPLC・蛍光検出にて血漿中tHcyを測定している．本方法は微量の検体量で簡便・迅速に測定可能であり，臨床的に非常に有用である．本稿では，各測定法を紹介し，われわれの測定した健康対象者ならびに症例での血漿中tHcy値を呈示する．

[ Summary ] Today the level of plasma total homocysteine (free homocysteine and protein-bound homocysteine;tHcy) is mostly used as a marker of folate and cobalamin functions and in cardiovascular risk assessment. Values for tHcy have been obtained by different methods, such as radioenzymatic assay, gas chromatography-masss pectrometry, high performance liquid chromatography (HPLC) and immunoassay. We have measured plasma tHcy concentrations of 130 normal control subjects and 2 homocystinuric patients, using HPLC of fluorescent compounds obtained after precolumn derivation. This method is clinically useful, since the sample volume is very small and it is quick and simple. In this section, methods for plasma tHcy determination, including ours, have been reviewed and the values of plasma tHcy concentrations of control subjects and patients has been shown.
