【 要旨 】 近年の薬物体内濃度測定技術の進歩や薬物動態学の発展によるtherapeutic drug monitoring (TDM) の普及は，これまでの経験的な薬物治療設計に新たな展開をもたらした．透析患者のような病態時には，患者個人間はもとより個人内においても，薬物動態パラメーターの変動がしばしばみられ，TDMを行い慎重に薬物動態を観察しながら，適切な薬物の投薬設計を行うことが望まれる．本稿では，薬物療法における薬物動態とTDMについて，その背景と活用の意義を振り返ってみた．

[ Summary ] Recent developments in therapeutic drug monitoring, which are based on the progress in measurement techniques for drugs in body fluids and the advances in our understanding of drugs'pharmacokinetics, has improved individualized pharmacotherapy. The pharmacokinetic parameters of drugs are often varied in patients with diseases such as renal failure. Therapeutic drug monitoring is effective in optimizing pharmacotherapy in such patients. We reviewed recent findings about pharmacokinetics and therapeutic drug monitoring in pharmacotherapy.
