【 要旨 】 緑内障を伴う透析患者に対する血液浄化法の工夫および薬剤投与治療について述べた.眼圧上昇を抑えるための血液浄化法の工夫としては,脳・血液関門と同様に眼・血液関門の存在を想定し,この仮説に基づき,不均衡症候群防止を目的とした各種血液浄化法を選択している.緑内障患者への血液浄化法として,当初は血液濾過法(HF)がおもに利用されていたが,その後は,置換液量を5~6l/回使用する血液透析濾過法(HDF)が好まれるようになっている.さらに今後は,acetate free biofiltration (AFB)の適応が大いに期待されている.

[ Summary ] The use of hemopurification methods and drug administration treatments for dialysis patients suffering from glaucoma were described. In the use of hemopurification to suppress intraocular pressure elevation, a blood brain barrier and blood eye barrier were presumed to exist. Based on this supposition, various hemopurification approaches were selected as a means to prevent the dialysis disequilibrium syndrome.
Hemofiltration (HF) was initially used as the main method of treatment for patients with glaucoma. The preferred treatment in these cases has come to behemodiafiltration (HDF), with 5 to 6 liters being replaced per session. These are high expectations that acetate free biofiltration will come to be a viable treatment in the future.
