| 【 要旨 】 | CAPDは今日，自己管理能力のある自立した患者のみでなく，日常生活に介護を要する患者も治療の対象としている．CAPDを安全かつ快適に継続するためには患者家族の理解と協力が不可欠である．要介護の患者では家族が実際の治療手技や患者の健康管理を行うため，家族の時間的拘東や精神的負担が大きい． 患者家族を支援するためには，患者および家族の不安や精神的負担を軽減する必要がある．無菌接合装置を利用したCAPDやAPDを選択することにより，手技に伴うストレスが軽減可能となる．慢性腎不全の保存期から患者および家族と話し合う機会をもち，医師による十分なインフォームド・コンセントとともに，専任ナースにより保存期から導入期，維持期を通して教育・指導を行うことは，患者および家族の不安を軽減し，良好な信頼関係を築くキーポイントと考えられる．導入期においては病棟のプライマリーナースと情報を交換しながら，一貫した指導を行うことも大切である．外来通院となってからは，電話を利用した相談が家族の負担軽減のために重要である． |

| [ Summary ] | Continuous ambulatory peritoneal dialysis (CAPD) is applied not only to independent renal failure patients, who are able to manage their own care but also to dependent ones. Understanding and cooperation by family members is essential for these patients to continue CAPD safely and comfortabiy. In addition, family members are faced with the restriction of free time in their own lives and consequentry mental stress, because they are forced to perform CAPD procedures and manage patient's health, in the case of dependent patients. In order to support patients' family members, medical staffs must reduce patients' uneasiness and mental stress as well as that of their family members. The choice of a sterile connection system in CAPD or an automated peritoneal dialysis (APD) systems may reduce their technically related stress. The concept of informed consent by the doctor in charge, and education and guidance by the primary CAPD nurse should be given to patients and their family members at the start of conservative therapy. Frequent communication with the same staff members is a key point in creating a reliable patient-staff relationship. Another important point is consistent guidance at the beginning of CAPD by the primary ward nurse in conjunction with the primary CAPD nurse. Consultations by telephone also help to ease the minds of out patients' families. |
