Since its inception, in June 1995 my staff at "the Nakagawa station" have visited patients in need of various types of care, including end-stage dialysis patients, and CAPD patients. We communicated closely with doctors and other hospital staff members.Through those experiences, we achieved the following successful results: patients had an excellent quality of life at home, both physical and psychological responsibility of family of patients decreased, patients returned home sooner because our staff cloud treat them at home, patients avoided re-hospitalization and patients were able to die in peace at home. |
