【 要旨 】 近年,透析医療の技術革新およびダイアライザーの高機能膜化に伴い,透析機器も多様化,インテリジェント化してきた.これらの機器を恒常的に安全に使用するためには,日々の保守点検を必要とする.当院では,これらの機器トラブルに対しては,臨床工学技士(以下,技士)による院内修理を原則とする.突発的なトラブルに対し,原因が明確で修理時間が15分以内であれば現場修理を行うが,それ以上の場合や原因不明なトラブルについては,メンテナンスルームにて修理を実施する.修理完了後,修理実施表に記録する.この実施表は機械1台ごとに備えており,過去の履歴を把握することで,トラブル原因の的確な判断資料となる.最終的には,定期的オーバーホールを含め,修理管理ファイル(修理内容,時間,交換部品,費用)に入力し管理する.技士が院内修理を実施することで,修理費の削減効果と,機器の稼働停止期間の短縮により,院内機器が有効的に機能すると考えられる.

[ Summary ] Technological developments in dialyzers and dialysis equipment have become more complex and varied in recent years. For safe and effective operation, daily maintenance checks are essential for each piece of equipment. In our hospital, these maintenance checks together with any repairs are the work of trained clinical engineers.
When the cause of the trouble is clear and simple, and the time estimated for the repair is less than 15 minutes, we repair the equipment of site in the dialysis room. However, when the cause of the trouble is not clear or the time estimated for the repair is longer than 15 minutes, the equipment is removed and then repaired in a machine maintenance room. Each and every problem is meticulously recorded on a maintenance record sheet attached to each piece of equipment.This procedure facilitates understanding the character of any specific equipment problem through examining the recorded past history of that piece of equipment. The details from the equipment maintenance record sheets are copied onto computer files. This allows centralized administration of all dialysis equipment in the hospital. This procedure has minimized the time needed for repairs of dialysis equipment, and has achieved cost and clinical efficiency.
