【 要旨 】 血液透析は，恒温で閉鎖系の中を流れている血液を透析液と接触させることにより濾過や除水を行っている．また，動静脈シャントを造設するなど本来の血流や血行路を変更して行う治療法でもある．このため，静脈系にさまざまな合併症が起こりやすいことが知られており，とくに肺循環系に及ぼす影響は重要である．本稿では透析における呼吸器合併症のなかで，致命的な合併症として知られる肺塞栓症について述べる．

[ Summary ] Pulmonary embolism is one of the most severe complications in hemodialysis patients. Ordinarily, blood circulates in the closed system and maintains a constant temperature. However, hemodialysis is administerd under extra-body circulation and unstable temperatures. Arterio-venous shunting also produces venous thrombosis and occurs pulmonary thrombosis. This is a review of the mechanism of pulmonary embolism under such special circumstances in hemodialysis.
