【 要旨 】 日本腎臓移植ネットワークを通じて，透析歴26.3年と21.0年の長期透析例が献腎移植を受けた．25年以上の透析症例の腎移植は世界的にもまれと思われる．両症例は，透析アミロイド症，二次性副甲状腺機能亢進症などによる骨・関節病変，深刻なシャントトラブル，常時低血圧，著しい動脈硬化などを抱えていた．ある意味で透析治療が行き詰まった状況でもあった．しかし，術後のQOLの改善はめざましく，これらの合併症があっても腎移植の障害にはならないと考えられた．一方，二十数年間という透析生活から術後の透析のない生活への環境変化は激しい．超長期透析から腎移植へ移行する場合には，合併症によるリスクの考慮と同時に，長年続いた生活様式の変化から生ずる精神的問題への配慮も必要である．

[ Summary ] We experienced 2 cases of cadaveric renal transplantation (RTx) who had been on hemodialysis (HD) treatment for 21.0 and 26.3 years. These cases had complications peculiar to long-term HD, such as dialysis-related amyloidosis, hypotension, shunt failure, secondary hyperparathyroidism and severe arteriosclerosis. They were in a condition that really requires an other mode of therapy like RTx. Successful RTx brought them an excellent quality of life in medical aspects. However,the sudden change in life style also caused psychological problems. We conclude that a history of very long-term dialysis and its complications does not interfere with RTx and the RTx will provide excellent QOL but psychological care is mandatory, especially in long dialysis patients.
