| 【 要旨 】 | 保存期腎不全における高血圧の管理は，腎不全の進行阻止にとって重要な要因となっているが，降圧薬の選択と使用規準には慎重を要する．また，透析患者の高血圧は，透析患者の死因の過半数を占める心・血管系の障害の最大の原因ともなっており，その予防と治療も重要である． 一方，透析低血圧の存在は透析患者に不快感を与えるばかりでなく，透析継続を困難とさせたり，高血圧の存在同様，重篤な心・血管系の病変を引き起こす原因ともなるので予防・対策が必要となる． |

| [ Summary ] | It is well known that hypertension degrades residual renal function in chronic renal failure (CRF), and causes atherosclerosis and cardio-vascular diseases in both CRF and hemodialyzed patients. Therefore, the management of hypertension in CRF and hemodialyzed patients is very important. All types of antihypertensive drugs can be used, but we must be aware of their pharmacokinetics and side effects. A hypotensive episode in a hemodialysis patient, often requires aggressive resuscitative measures, premature termination of hemodialysis, and sometimes causes severe cardio-vascular disorders. To prevent hypotensive episodes, it is essential to set the adequate so-called dry weight for an individual patient. |
