[ Summary ] The most common major complication after colonic polypectomies is bleeding and perforations. To prevent these incidents must change the environment for endoscopic polypectomies, bowel preparation, and maintenance of endoscopic patients. We must perform appropriate techniques for performing polypectomy and clipping after performing polypectomy properly. We evaluated the usefulness of prophylactic clipping after polypectomy with a randomized controlled trial. Post polypectomic bleeding occurred from 2 lesions (1.2 %) in the clip group and 7 lesions (5.4 %) in the non-clip group (p< 0.01). Prophylactic clipping decrease the occurrence of post polypectomic bleeding.
We examined 122 cases of post polypectomic bleeding in our hospital. In all the cases with bleeding after polypectomy, except one, we could achieve hemostasis with endoscopic procedures. The clipping procedure is usefull to achieve hemostasis. Although severe bleeding can not be controlled completely with clip ligation, hemostasis is achieved with a detachable snare.
