[ Summary ] | Type V pit pattern, using magnifying endoscopy, is divided into the categories of VI (irregular) and VN (non-structure). The former represents intramucosal cancer, which are slightly and deeply invasive cancers into the submucosa, whereas the latter represents deeply invasive cancers into the submucosa. We classfied VI into two groups, studied the diagnostic ability of invasion depth for early colorectal cancers and the relationship between the subclassification of VI and histological findings. The following results were obtained : (1) There was a high level of diagnostic ability and accuracy, sensitivity, specificity as well as positive predictive value in cases where the carcinoma had invaded the submucosal layer by more than sm2 or 1,000 μm. (2) The subclassiflcation of type VI pit had a high correlation with the degeneration and disappearance of crypts, the state of muscularis mucosae and desmoplastic reaction. On the basis of our results (1) and (2), the subclasssification of type VI pit was established as an effective method to achieve higher diagnostic accuracy. |
