Recently, several endoscopists have tried to apply a newly developed endoscopic resection technique (submucosal dissection method) for colon and rectal mucosal cancer. EMR, using IT knife, needle knife, hook knife and flex knife etc, has resulted in curative treatments and provided precise information for histological examinations as well as EMR for early gastric cancer. These techniques may involve increased risk of bleeding and perforations compared with conventional methods because of the electrical characteristics of the colon. Improvements in electrical high-frequency surgical units for cutting and coagulation, new devices for hemostasis, and submucosal injection materials enable us to perform cutting and dissecting EMR in a safe manner. Further improvements are needed to make this technique safer and more reliable as a standard endoscopic method for large colorectal tumors. |
