| [ Summary ] | Irregular pit pattern is characterized by its complex shape and plays an important role in magnifying endoscopic diagnosis of early colorectal cancer. However, little has been clarified so far especially from the viewpoint of the correlation between pit patterns and histological findings. In this study, the complexity of pits and corresponding histological, glandular structures were determined by use of fractal dimensions(FD). Their correlation and their relationship to the histological grade of atypia and vertical depth of submucosal invasion were examined. As for pits, mean FD values differed significantly between each pattern, with round, roundish, tubular and branching patterns. The result was correlated with our sense of complexity in these cases. FD values of pits were significantly correlated with those of glandular structures. The higher the FD value of pit pattern, the higher the complexity of the histological glandular structure and the histological grade of cellular atypia. However, no significant correlation was found between the FD of pits and the vertical depth of submucosal invasion although more than a half of the lesions with massive submucosal invasion turned out to be those with FD values over a high point of 1.4. These results suggest that the capacity of irregular pit pattern which diagnoses early colorectal cancer depends on its correlation to the complexity of glandular structures and the histological grade of atypia. |
