Title Colorectal carcinomas with submucosal invasion : The analysis of radiographic characteristics in comparison with pathologic PG-NPG classification

[ Summary ] We investigated the radiographic characteristics of early colorectal carcinoma with submucosal invasion (CCSI) in double contrast images. We divided 196 CCSI lesions into protruding and depressed types, furthermore, sub-classifying these into lobular and smooth types was done on the basis of surface structure. Each type was analyzed in comparison with pathologic findings for resected specimens. Almost all CCSIs could be identified as protruding lesions in X-rays, these having a smooth rather than a lobulated surface demonstrating greater malignancy, despite a smaller size. The proportion of pathologic non-polypoid growth type was observed 72% in sessile lesions with smooth surface, however, 28% in those with lobulated surface. These findings suggested that the greater part of sessile lesions with smooth surface was considered to be superficial depressed type origin in colorectal cancers. It is clinically important to discriminate these from other polypoid lesions in establishing patient treatment. Representative two lobular and two smooth lesions were demonstrated.
