【 要旨 】 | 大腸Ⅱcの現状と存在意義を再確認するため,クリニックで内視鏡で拡大観察され切除された大腸腺腫と早期癌11,296病変を発育形態分類に準じて解析した.隆起型5,934例,平坦型5,255例,陥凹型107例に分類し,臨床病理学的に比較検討を行った.陥凹型の病変頻度は1.0%であった.T1癌の頻度は,隆起型0.5%と平坦型0.4%に対して,陥凹型は21.5%と有意に陥凹型で浸潤傾向が強かった(p<0.0001).また陥凹型に関しては,ⅡcとⅡc+Ⅱa 3.3%,Ⅱa+Ⅱc 26.6%,Ⅰs+Ⅱc 38.5%とT1癌の頻度が上昇し, 腫瘍径が10mmを超えると80.0%と高率で浸潤癌の頻度が高くなり,これまでの報告同様陥凹型の悪性度が再確認された.画像強調内視鏡診断やAI診断の今の時代に病変の陥凹形態を慎重に観察していく診断体系を改めて見直し,次の内視鏡の世代へと継承していく大腸Ⅱcの存在意義が確認された. |

[ Summary ] | To confirm of the basis for the existence of depressed-type colorectal tumors, we analyzed 11,296 adenomas and early colorectal cancers that were detected via magnifying colonoscopy and resected. The lesions were classified as the protruded type (n=5,934), flat type (n=5,255), and depressed type (n=107) according to their developmental morphology and compared clinicopathologically. The incidence of the depressed-type tumors was 1.0 %. The rate of T1 cancer was 21.5 % in the depressed-typed tumors compared with 0.5 % in the protruded-type and 0.4 % in the flat-type tumors (p<0.0001). In the depressed-type tumors, the rates of T1 cancer in type IIc and IIc+IIa, type IIa+IIc, and type Ⅰs+IIc were 3.3 %, 26.6 %, and 38.5 %, respectively. Thus, we reconfirmed the malignant potential of depressed-typed colorectal tumors, as previously reported, and verified the basis for their existence at the time of diagnosis using image-enhanced endoscopy or artificial intelligence. |
