【 要旨 】 抗TNF-α抗体製剤を中心に手術への影響や術後治療の考え方について解説する．手術への影響を複数の研究が報告しメタ解析も行われている．潰瘍性大腸炎，クローン病ともに術前の抗TNF-α抗体製剤の使用は影響していないとする報告がやや優位であるものの，施設や解析方法によっては周術期感染合併症のリスクとなる結果も出ている．最近のメタ解析では影響がなく，現在のところ術後経過を危惧して治療をためらう必要はなさそうである．術後治療については決まったタイミングや適応はないが，導入時と同じく術後合併症を含めた感染症を否定しておくことが必須である．

[ Summary ] Anti-tumor necrosis factor-alpha (TNF-α) treatment is effective against IBD. Although several meta-analyses have been performed to evaluate the association between TNF treatment and surgical morbidity, the results are controversial. A recent meta-analysis showed a negative association between preoperative TNF treatment and surgical morbidity. Currently, there are no guidelines that restrict TNF treatment after surgery. However, we should confirm the absence of postoperative infectious complications prior to TNF treatment.
