【 要旨 】 内視鏡的バルーン拡張術は，クローン病の小腸狭窄病変に対する治療として症状改善，手術回避に対して効果的であることが証明されている．そのため内視鏡的バルーン拡張術は手術に代わる手段として，国内外の炎症性腸疾患診療ガイドラインでも推奨されている．しかしながら慢性炎症および術後変化による屈曲や癒着から，すべての患者が拡張術の恩恵を受けるわけではない．本稿では，そのような治療困難例に対する技術的な工夫を紹介する．

[ Summary ] EBD has emerged as an alternative to surgery and has been proven effective for the treatment of small intestinal strictures in CD. EBD is therefore among the recommended procedures for intestinal strictures in the current CD practice guidelines. However, procedures are not always successful because of bending or adhesions caused by chronic inflammation or postoperative structural changes. Herein, we describe our approaches to overcome technical difficulties.
