【 要旨 】 消化管病変を伴う膠原病，血管炎には多くの疾患がありその病態は血流障害によるものである．血管炎は罹患血管のサイズにより病因，病態が異なり結節性多発動脈炎と抗好中球細胞質抗体関連血管炎とに分類される．病態の主体はおもに閉塞性血管炎による虚血性変化と潰瘍病変や動脈瘤破裂による出血である．これらの疾患はそれぞれに特徴がありIgA血管炎のように皮膚病変が消化器症状に先行して出現するものもあれば腹痛や下血，血便を主訴に医療機関を受診するケースもあり，各疾患の特徴を理解し，これらの疾患を常に念頭におくことにより正しい診断・治療に導けるものと思われる．

[ Summary ] Collagen disease and vasculitis cause gastrointestinal lesions and the pathology is due to blood flow disorders. Vasculitis with gastrointestinal involvement demonstrates ulceration or perforation of the gastrointestinal tract owing to transmural ischemia caused by necrotizing focal segmental vasculitis. Vasculitis differs in etiology and pathology depending on the size of the affected blood vessel and is classified into polyarteritis nodosa and antineutrophil cytoplastic antibody.
