【 要旨 】 FITを用いた大腸がん検診は,日本を含めて広く世界的に実施されている.FITは非侵襲的検査でありながら,大腸癌に対して比較的優れた診断精度を有するが,それでも一度のFIT検査のみでは,大腸癌を見逃す可能性がある.しかし,これを理由にFIT検診を否定することは不適切である.FIT検診は,FIT検査を繰り返し行い,見逃しリスクを減らすことで成り立つ検診プログラムであり,FIT検診はプログラム全体としては非常に優れた検診法といえる.ただし,FIT検診による日本の対策型検診に,大腸内視鏡検査を現状よりも積極的に取り込むことについては検証の余地があり,最初から大腸内視鏡を行う内視鏡検診の導入を含めて,さらなる議論が望まれる.

[ Summary ] Fecal immunochemical test (FIT)-based colorectal cancer (CRC) screening is performed across several countries, including Japan. For the population-based CRC screening in Japan, FIT is used as a primary screening tool and total colonoscopy is performed for individuals with positive FIT results. FIT has strengths of its noninvasiveness and good diagnostic ability for CRC. However, the screening sensitivity of FIT for CRC is not perfect, and, CRC may be missed with a single FIT. Thus, repeating FIT screening (annually in Japan) is essential, and the utility of FIT-based screening should be evaluated not by the performance of a single FIT but by that of the whole screening program, including repeated FIT screening. In this sense, FIT-based screening is an effective program when considered in totality. However, in order to increase the effectiveness of CRC screening, more aggressive use of total colonoscopy in the screening, including the use of total colonoscopy as a primary screening tool, is worth consideration. We need to discuss the optimal methodology to incorporate total colonoscopy into the FIT-based screening protocol to further reduce CRC mortality and incidence.
