[ Summary ] |
A female in her 60's with a colonic lesion of the sigmoid colon was treated by ESD. The preoperative colonoscopy showed no findings of deep submucosal invasion. After ESD, histological diagnosis was T1b (SM 1,000μm), Ly0, V0, HM0, VM0 with IHC such as desmin, D2-40 and EVG. After repeated discussion including the colorectal surgeon, the patient decided to followed up without surgical intervention. Annual colonoscopy, tumor marker, and CT examination was performed. Five years after ESD, the tumor marker was increasing and subsequent CT scan revealed a mass near the sigmoid colon. A colonoscopy also found a SMT (sub-mucosal tumor) like lesion. Additional surgery revealed sub serosal carcinoma cells without mucosal or submucosal invasion. |
