| 【 要旨 】 | 腸管感染症のほとんどは対症療法のみで自然軽快するが，時に診断を迫られることがある．確定診断には病原体の検出が必須であるものの，その同定率は決して高くない．病歴や臨床経過に加え，特徴的な画像所見を拾い上げていくことで，起因病原体を推定し，鑑別疾患を絞り込む努力が必要である．本稿では，腸管感染症と臨床像の類似する急性炎症性腸疾患（虚血，血管炎，薬剤）について概説する． | |

| [ Summary ] | Most infectious colitis is naturally improved by symptomatic treatment, however, it is sometimes difficult to make a definitive diagnosis. Although it is imperative to detect pathogens to confirm the diagnosis, identification of each pathogen which causes infectious colitis still remains difficult. It is necessary to identify infecting antigens and differential diagnosis by their typical image findings as well as history and clinical course. This article describes the difference between intestinal tract infections and acute inflammatory bowel disease in which clinical features are similar to infectious colitis. |
