【 要旨 】 見逃しのない精度の高い大腸内視鏡検査を行うために，当センターで経験した，3年以内に検査歴のある経年発見進行大腸癌を見逃し例として25例を対象に観察撮影法について検討した．見逃し例の検討から，上行結腸の深いひだ間に対しては1ひだごとに周回しながら観察，屈曲部に対しては腸管を伸展して挿入・抜去を行い観察，多発憩室のある伸展不良の腸管に対してはスコープが抜けないように保持してゆっくり観察することが必要である．

[ Summary ] To investigate large intestine endoscopy with high precision, the clinical features of 25 patients with large intestine advanced cancer were examined.
Careful observation of deep fold intervals in the ascending colon, as well as full observation of intestinal tract flexures (hepatic, splenic, SDJ, RSJ is necessary). It is a also necessary to stop movement and observe the sigmoid colon due to frequent involvement of the diverticulum.
