【 要旨 】 日本ではクローン病（Crohn's disease ; CD）は約4万人となり，増加の一途をたどっている．小腸内視鏡やカプセル内視鏡といった小腸観察も容易になってきたが，時に診断に難渋する症例も少なくない．CD診断の際に鑑別が問題となる代表的な疾患を挙げて，特徴的な臨床像，とくに内視鏡などの画像所見を中心に鑑別のポイントを解説した．炎症性の腸疾患の診断は，特異的な所見が確認される場合はまれであり，鑑別困難例の最終的な確定診断のためには，症状や家族歴，腸管外の全身徴候，種々の内科治療に対する反応性や治療経過といったすべての臨床所見を総合的に判断することが重要である．

[ Summary ] The number of Crohn's disease patients has increased in Japan. It is sometimes difficult to diagnose Crohn's disease. Differential diagnoses are enumerated and characteristics are explained in terms of clinical evidence and endoscopic findings.
