| 【 要旨 】 | Cold polypectomyは一見簡単な手技ではあるが，遺残や後出血を防ぐためには多少の留意点が必要となる．大型生検鉗子を用いるCold forceps polypectomy（CFP）は，鉗子を半開きにして病変径に合わせることが重要である．方向が合わない場合には鉗子全体を回転させ，病変が先端外径（2.8mm）より大きい場合は複数回の摘除を考慮する．Cold snare polypectomy（CSP）は腫瘍の残存がないように正常粘膜も含めて切除する必要がある．後出血は少ないとされるが，出血が止まりにくい場合には，CFPでは出血点を鉗子で把持し，CSPでは出血部または周囲の粘膜を含めて再絞扼を行う圧迫止血術も考慮する． | |

| [ Summary ] | Cold polypectomy is usually a simple technique, but special care must be taken in certain circumstances. When undertaking Cold Forceps Polypectomy (CFP) using jumbo forceps, it is important to adjust the jaws such that they are partly open and match the polyp diameter, and to rotate the forceps completely from the handle. If the polyp is larger than the tip of the jumbo forceps, it is recommended to resect it twice. In cold snare polypectomy (CSP), peripheral normal mucosa should be resected, and in rare cases can result in delayed bleeding. It is possible to stop initial bleeding by forceps grasping or by re-snaring the area with surrounding mucosa. The pressure applied using these techniques promotes coagulation and thus hemostasis. |
