Recent advances in endoscopic and pathological diagnosis, which include the simple mucosal biopsy criteria, have enabled us to reliably differentiate inflammatory bowel disease (IBD) from other forms of colitis, and Crohn's disease from ulcerative colitis. On the other hand, many problems with diagnosis remain. Patients with IBD frequently take biological preparations having carcinogenesis and a compromised risk. An endoscopic and histologic standard to recommend dosage end time is necessary. However, related studies are in the early stages. In addition, pathological diagnosis of the disease group termed monogenic IBD due to unifactorial variation has become a new issue. Clinical courses and endoscope findings are often similar to those for IBD. However there are few reports on generalized biopsy features from the viewpoint of pathological diagnosis. ANCA-associated vasculitis often exhibits IBD-like enterocolitis. Biopsy derived diagnoses should also be the subject of future research. We touch on the above-mentioned subjects and others concerning the pathological diagnosis and pitfalls of microscopic colitis, idiopathic mesenteric phlebosclerosis, and chronic nonspecific multiple ulcers of the small intestine. |
