Theme The roll of superficial depressed type on developmental process of colorectal cancer
Title A case of IIa+IIc type submucosal invasive colon cancer followed for one year

[ Summary ] A case of IIa+IIc submucosal invasive (sm) cancer was followed using endoscopy for about one year. The patient was a 56-years-old woman. She was presented for a work-up because she had a positive reaction on the stool occult blood test in a mass screening test. On June 29. 1995 colonoscopic examination revealed a very slightly elevated IIa+IIc-like lesion contralaterally to the Bauchin's valve. Endoscopic mucosal resection (EMR) was recommended. About 2 months later, the patient received a colonoscopic examination at an other hospital, of her own accord. Examination at the other hospital failed to detect the IIa+IIc-like lesion. On July 17, 1996, about 1 year after examination at our institution, the patient returned there for a work-up. Colonoscopoic examination disclosed a deeply depressed lesion of higher elevation than before, accompanied by bleeding and erosion at the same site. After histopathological exammation of an EMR specimen a diagnosis of sm2 moderately differentiated adenocarcinoma was made, and curative bowel resection with lymph node dessection was performed. This case seems to provide infomation of interest from the standpoint of growth and progression of colonic cancer.
