【 要旨 】 Clostridium difficile感染症の症状と内視鏡を中心とした画像診断について述べた．症状では下痢，腹痛，発熱がみられることが多いが，下痢がない場合がある．内視鏡所見として典型例ではさまざまな程度の偽膜がみられるが，偽膜がないことも少なくない．他の所見としてアフタ様病変が報告されているが，小白点がみられることも多く，まれに正常像が観察される．腸管洗浄液を用いた前処置によって偽膜が剝落し所見が修飾されている可能性がある．非典型例を誤診しないためには内視鏡像の多様性を理解したうえで本症を想起する必要がある．

[ Summary ] We describe the symptoms and imaging diagnosis (mainly endoscopic findings). The main symptoms are diarrhea, abdominal pain, and fever. However, diarrhea is sometimes absent. Representative endoscopic findings are pseudomembranes of various degrees but are not necessarily observed. Aphthoid lesions have been reported as alternative findings, but colonoscopy often reveals white spots and rarely normal appearance. By the use of a lavage solution, pseudomembranes may be exfoliated. To avoid misdiagnosis, examiners should be aware of diverse endoscopic findings.
