【 要旨 】 炎症性腸疾患に付随する炎症関連大腸癌は散発性大腸癌と比較し,進行が速く,予後不良な癌である.しかし炎症関連癌におけるその発癌様式と癌形質獲得の関連はこれまで明らかとされていない.本稿では,癌の病理学的特徴から上皮細胞の分化制御機構に着目し,炎症による分化制御の破綻が炎症関連大腸癌の癌形質と密接に関わることを概略する.

[ Summary ] Patients with inflammatory bowel disease (IBD) have an increased risk of colitis-associated colorectal cancer (CAC). CAC often acquires chemoresistance, resulting in poorer prognosis than that of sporadic colorectal cancer (CRC). The mechanism by which CAC enhances malignant potential remains unknown. In this issue, we focus on whether the pathological characteristics of CAC that result in enrichment of mucinous carcinoma are related to regulation of intestinal differentiation. Deregulation of the stability of proteins including Atoh1 has been shown to be associated with the mucinous form and higher malignant potential of CAC.
