【 要旨 】 志賀毒素産生性大腸菌（Shiga toxin-producing Escherichia coli；STEC）の代表であるO157 STECとそれ以外のnon-O157 STEC腸炎の臨床像を述べた．O157 STEC腸炎では，特徴的な内視鏡所見を呈する例が多い．すなわち，炎症所見は右側結腸で著しく，左側結腸に移行するに従い漸減する．また縦走潰瘍がみられる例もある．一方，これまでnon-O157 STEC腸炎の内視鏡所見についての詳細な報告はない．われわれが経験した虚血性大腸炎類似の内視鏡所見を呈したO55 STEC腸炎症例を提示した．non-O157 STEC腸炎の内視鏡所見については，今後の症例の積み重ねが期待される．また，2011年にドイツで起こったO104 STEC感染症の集団発生について文献的に概説した．

[ Summary ] Clinical features of hemorrhagic colitis associated with O157 Shiga toxin-producing Escherichia coli (STEC) and non-O157 STEC are reviewed. Hemorrhagic colitis associated with O157 STEC presents characteristic colonoscopic fi ndings associated with the inflammatory gradient and longitudinal ulcers. On the other hand, little is known about the colonoscopic findings accompanying with hemorrhagic colitis associated with non-O157 SETC. One case of hemorrhagic colitis associated with non-O157 STEC (O55) is presented. The colonoscopic findings resembled those of ischemic colitis. Additionally, the German O104 STEC outbreak in 2011 is outlined from medical literature.
