【 要旨 】 下痢，血便，腹痛，発熱などの大腸炎症状を認める症例の診療では，症状・病歴の詳細な問診，血液学的検査，画像所見によって総合的に診断していく．そのなかでも大腸内視鏡検査の意義は，診断確定・活動性評価・治療方針決定など多岐にわたるため，各疾患の特徴的な内視鏡所見を熟知しておく必要がある．大腸炎症性疾患は感染性と非感染性に大別され，まずはじめに便細菌培養検査を提出することが重要である．

[ Summary ] The diagnosis of colitis for patients with symptoms such as diarrhea, hematochezia, abdominal pain, and fever is based on the evaluation of clinical features, laboratory data, radiological findings, and endoscopic features. Colonoscopy is a mandatory procedure for the initial diagnosis, the assessment of severity, and the healing of colitis. It is important for endoscopists to understand the characteristic endoscopic findings associated with various types of colitis. This is especially true in developing initial diagnoses or excluding infectious colitis.
