【 要旨 】 大腸上皮性腫瘍病変に対する内視鏡下摘除において，ESDとEMR，とくに分割EMRの住み分けは治療の根治性および安全性の面から重要である．ESDを完遂できる技量を有する内視鏡医は，ESDによる内視鏡下摘除を選択し，そうではない内視鏡医は，病変の性質（病変部位，肉眼形態，良悪性，癌の量，予想される線維化の有無など）や術者の技能から，その時点で適切と判断する手技を選択すればよいと思われる．またESDを選択した際には，必ず上級医の管理下において治療を行うべきである．

[ Summary ] EMR is still the primary method for treatment of colorectal epithelial neoplasia. However, ESD has recently come to be widely performed. We must comprehend the features of both therapeutic methods while improving our skills. At the same time, we must select the most appropriate method for treatment of colorectal epithelial neoplasia. In order to judge which is appropriate ESD or EMR for the treatment of colorectal epithelial neoplasia, we must observe the lesions carefully with conventional endoscopy and magnifying endoscopy to determine microvascular patterns, microsurface patterns and pit patterns.
