Title Differential diagnosis of superficial depressed type lesions of the colorectum by radiographic flndings with double contrast barium enema

[ Summary ] The diagnostic capacity of barium enema was evaluated in the differential diagnosis of colorectal depressed lesions, i.e., the discrimination among superficial depressed neoplasms (adenomas and intramucosal carcinomas), hyperplastic nodules and aphtoid lesions of inflammatory disease.
There were three useful findings in discrimination by barium enema, that is a central bariurm collection, multiplicity and the arrangement of lesions. Many of the depressed neoplasms showed a round radiolucent halo with a faint barium collection which was not present in hyperplastic nodules. However, seven (18%) of 39 depressed neoplasms did not demonstrate a central barium collection. No difference was noted in the features of central depression between adenomas and intramucosal carcinomas. With regard to multiplicity, most of the depressed neoplasms were solitary, whereas hyperplastic nodules and aphtoid lesions were multiple. As for the arrangement of lesions, hyperplastic nodules were distributed in the rectum and sigmoid colon, while aphtoid lesions in Crohn's disease tended to be longitudinally aligned.
These results suggest that discrimination is possible among depressed neoplasms, hyperplastic nodules and aphtoid lesions, but not between adenomas and carcinomas, based on barium enema findings.
