【 要旨 】 Barrett食道（BE）の臨床的意義は発癌リスクを有する点であり，近年，欧米の白人男性では食道腺癌（EAC）の頻度が増加し50％を超え注目されるようになった．BEの定義については，近年，国際的なコンセンサスが得られたものの，その有病率は不明な点が多い．わが国のshort-segment BE（SSBE）の頻度は15.8％（95％CI 15.0～29.5），long-segment BE（LSBE）の頻度は0.3％（95％CI 0.3～0.7）で，LSBEからの発癌頻度は年率1.2％とされている．組織学的にdysplasiaの程度を評価することは難しい場合があるが，治療指針決定にはきわめて重要である．BEのサーベイランスは，定期的な内視鏡観察が行われるが，high grade dysplasiaに対してはより短い検査間隔が推奨される．
