【 要旨 】 胆膵疾患は，良悪性の鑑別が時に困難である．内視鏡的逆行性膵胆管造影検査（ERCP），超音波内視鏡（EUS）といった内視鏡を用いた診断技術が発展しており，近年では，共焦点レーザー内視鏡（CLE）を用いることで，細胞レベルで組織を直接視覚化でき，診断における上乗せ効果が期待できるようになってきた．現在使用されているCLEは，内視鏡の鉗子チャンネル内に挿入して用いるプローブタイプのprobe-based CLE（pCLE）である．胆道疾患に対しては，ERCPによる胆管狭窄診断における有用性が報告されており，膵疾患に対しては，おもに膵嚢胞性腫瘍の診断における有用性が報告されている．本邦でも今後有用性の評価をすべくさらなる普及が望まれる．
