【 要旨 】 近年，脂肪肝の新しい疾患概念metabolic dysfunction-associated fatty liver disease（MAFLD）が22カ国32名の専門医より提唱された．MAFLDは，肝線維化や肝発癌のリスク因子である肥満や糖尿病などの代謝異常を組み入れ基準としている．また，MAFLDは，nonalcoholic fatty liver disease（NAFLD）の除外基準とされていた飲酒量やウイルス性肝疾患とは独立した疾患概念であり，アルコール性肝疾患やウイルス性肝疾患における脂肪肝の意義についても検討可能である．本稿では，MAFLDの肝線維化診断能について概説する．さらに，肝癌患者におけるMAFLDの実態と早期発見のためのスクリーニングについて，近年の研究報告を基に論述する．
