【 要旨 】 全世界的な肥満人口の増加に伴い，非アルコール性脂肪性肝疾患（NAFLD）は今後，肝細胞癌の原因の中心となると考えられる．早期発見や発癌予防薬，抗癌剤の開発のため，NAFLDからの発癌メカニズムの解明が急務である．PNPLA3などの遺伝的要因と，食事習慣などの環境要因が複雑に関与することで多面的に肝脂肪沈着という表現型を呈する．NAFLDから発癌に至る経路として肝内の酸化ストレスの増加，オートファジーの機能異常，小胞体ストレス，胆汁酸代謝異常などさまざまなメカニズムが動物モデルで報告されている．本稿ではNAFLDを中心に代謝異常による非B非C型肝発癌メカニズムについて総説する．
