【 要旨 】 大腸ポリープ病変に対するcold polypectomyは，高周波装置による通電を伴わないことで偶発症を低減できるものの，組織への熱焼灼がないために腫瘍の遺残とその後の再発が懸念されている．最近の研究では，cold polypectomy後の遺残・再発の割合は1～4％と低く，従来の高周波装置を用いたhot polypectomyやEMRと同等の成績を示している．一方で，cold polypectomyの適応を超えるような大きな病変では遺残・再発のリスクが高まることも報告されている．cold polypectomy後の遺残・再発を低減するためには，①病変サイズに応じた適切な治療法の選択，②専用デバイスの使用，③病変周囲の正常粘膜を含むスネアリング，④画像強調観察や色素散布観察を用いた粘膜欠損部の確認，の四つの手順が重要である．
