| 【 要旨 】 | Cold polypectomyは，高周波を用いないポリープ切除法であるため，手技の過程は従来の高周波を伴う切除法よりも簡略かつ短時間で病変切除が完了される．さらには，高周波に関連するとされる遅発性の出血や穿孔のリスクがきわめて低く安全性の高い手技である．一方で，cold polypectomyにおける浅い切除深度や切除断端の病理組織学的評価の困難性などの問題から，早期癌はその適応外とされるべきであり，cold polypectomyを実施するには正確な術前の質的診断が求められる．以上から，大腸と十二指腸の腺腫がcold polypectomyの適応であり，cold polypectomyが手技として確立しつつある現在，より高いエビデンスを基にしたcold polypectomyの診療ガイドラインのupdate が望まれる． | |
