【 要旨 】 胆石性膵炎の診断はほかの成因による膵炎と治療方針が異なるため，急性膵炎の診断時に速やかに行うことが重要である．急性膵炎ガイドラインでは急性胆石性膵炎のうち，胆管炎合併例，胆道通過障害の遷延を疑う例には，早期の内視鏡的逆行性胆管膵管造影（ERCP）/内視鏡的乳頭括約筋切開術（EST）が推奨されている．一方，ERCP/ESTは偶発症発生頻度の高い手技であり，早期の介入は膵炎をさらに悪化させる可能性があるため，適応を慎重に行い十分な診療体制とinformed consentを得たうえで施行することが望ましい．
