【 要旨 】 A型胃炎は成因から自己免疫性胃炎とも呼ばれ，抗胃壁細胞抗体の出現による胃体部優位の萎縮を特徴とし，無酸症と高ガストリン血症を呈し，悪性貧血の原因となり，胃NETと胃癌の発生母地となる．A型胃炎の内視鏡所見はいわゆる逆萎縮パターンが特徴的とされるが，固着粘液やwhite globe appearance（WGA）などの所見も高頻度にみられる．生検により病理組織学的に胃体部優位の粘膜萎縮や endocrine cell micrones（t ECM）などを確認することが診断に有用となる．胃癌を合併するA型胃炎症例はやや高齢で高度萎縮例が多く，胃癌発生はA型胃炎の晩期合併症の一つに位置付けられる．A型胃炎を背景とした胃癌は早期癌が大部分を占め，組織型は分化型が多く肉眼型は表面隆起型を呈することが多い．
