【 要旨 】 超高齢社会を迎え，血栓塞栓症の発症リスクの高い高齢者に対して，内視鏡診療を行う機会が増加している．抗血栓薬は，抗血小板薬，抗凝固薬，血栓溶解薬に大別される．従来の消化器内視鏡診療では抗血栓薬の休薬を推奨していたが，最新のガイドラインにおいては，休薬をしないという選択肢も認められるようになった．しかし，ガイドラインに記載されているステートメントのエビデンスレベルは低いものが多く，今後とも本分野におけるevicence levelの向上を意識した臨床研究が必要である．

[ Summary ] Owing to the aging population in Japan, we have to perform gastrointestinal endoscopy in the patients with high thromboembolic risks. Antithrombotics are divided into antiplatelets, anticoagulants, and thrombolytics. The previous guidelines regarding gastrointestinal endoscopy in patients with antithrombotics recommended stopping the agents, but the newest guidelines allow practitioners to perform gastrointestinal endoscopy without cessation of the agents. The evidence levels of the statements, however, are quite low, and further active clinical research regarding this topic should be actively conducted in future.
