【 要旨 】 近年，巨大な総胆管結石や積み上げ結石に対して，直径12mm以上の大バルーンを用いるEPLBD（endoscopic papillary large balloon dilation）が開発され，その有用性が多数報告され，本邦も含めた全世界で普及している．そして2017年に，EPLBDを安全かつ確実に実施するために「EPLBD診療ガイドライン」が日本消化器内視鏡学会より上梓された．本ガイドラインでは，定義と適応，手技，特殊な症例への対処，偶発症，治療成績，術後経過観察の六つの項目についての21個のClinical Question（CQ）が作成され，本稿ではこのガイドラインの要点とその実際について述べる．

[ Summary ] Recently, endoscopic papillary large balloon dilation (EPLBD) developed as a procedure of biliary stone removal using more than 12 mm in diameter for large or multiple stones. It has become widely used in the world after the many reports about the effectiveness of EPLBD. However, there have been no guidelines for safe and accurate use of EPLBD to date. Therefore, the Clinical Practice Guidelines for EPLBD was reported by Japan Gastroenterological Endoscopy Society (JGES) in 2017. The guidelines were divided into six sessions: (i) indications, (ii) procedures, (iii) special cases, (iv) procedure‒related adverse events, (v) treatment outcomes, and (vi) postoperative follow up observation with 21 clinical questions.
