| 【 要旨 】 | 大腸腺腫に対する内視鏡的ポリペクトミーは，大腸癌の発生率，死亡率を低下させるうえで重要である．従来はおもにhot snare polypectomy（HSP）が行われてきたが，近年，高周波通電を用いないcold snare polypectomy（CSP）における偶発症リスクやコストの低さ，抗血栓薬内服中の患者に対しても偶発症リスクが増加しないなどの有用性が周知され，本邦でも普及しつつある．CSPは安全性や有用性の面から非常に優れた治療手技であるといえるが，その適応病変は限られており，適応外の病変にはこれまでと同様にHSPを行うことになる．6～10mmの病変に対するCSPとHSPそれぞれのメリット・デメリットを理解し，病変に応じて適切な治療手技を選択することが重要である． | |

| [ Summary ] | Endoscopic polypectomy for colorectal adenoma is important in reducing the incidence and mortality of colorectal cancer. Hot snare polypectomy (HSP) has historically been performed for colorectal polyps. Recently, cold snare polypectomy (CSP) without the use of electrocautery is spreading because its advantages have been publicized, a low risk for the patients during antithrombotic drug use, lower risk of complications, and lower cost of CSP. Cold polypectomy is a very excellent procedure from the point of view of safety and the utility. However, each lesion must be assessed, and a lesion that does not meet CSP criteria will be given HSP as before. It is important that we understand the pros and cons in each procedure, and that we choose appropriate procedures depending on a lesion. |
