[ Summary ] |
Endoscopic ultrasonography (EUS) has high local resolution and is useful for diagnosis of gallbladder lesions. Diagnosis of gallbladder cancer is easy if the basal section is broad with nodular elevated lesions and internal echoes are low echo without echo free areas. However gallbladder cancers show various macroscopic findings, differentiation from other benign diseases is often a problem. When advancing the diagnosis of gallbladder lesions, we should pay attention to whether it is a raised lesion, a wall thickening lesion, or a filled/mobilized lesion. When the gallbladder wall is defined with EUS, it is drawn in two or three layers. Irregularities and tears in the wall structure are indicators of advanced cancer. However it is difficult to accurately distinguish the invasion depth of cancers except for lesions that are clearly pedunculated. In the future, there will be an accumulation of cases by using contrast harmonic EUS. Establishment of diagnostic criteria is expected for gallbladder lesions, which are difficult to obtain from pathological specimens. |
