CT and MRI/MRCP diagnosis of intraductal papillary mucinous neoplasm (IPMN) of the pancreas and the differential diagnosis of IPMN vs. other neoplastic and non‒neoplastic pancreatic cysts were described. Imaging findings of IPMN reflect cystic dilatation of branch and/or main pancreatic ducts (MPD) owing to mucin hypersecretion. The likelihood of malignancy is assessed by indirect findings, including cyst size (≥3 cm), diameter of the MPD (≥5 mm, ≥10 mm), the presence/absence of mural nodule and its size (≥5 mm), and the presence/absence of MPD stricture. Care should be taken not only for the development of invasive carcinoma derived from IPMN but also invasive carcinoma concomitant with IPMN. It is important to be aware of diagnostic pitfalls of IPMN especially for the differential diagnosis of IPMN vs. serous cystic neoplasm (SCN). |
