【 要旨 】 粘膜下腫瘍（submucosal tumor；SMT）は上下部内視鏡検査において偶然発見されることが多い．病変の主座が粘膜下に存在し正常上皮で覆われているため，通常の内視鏡下観察のみでの画像診断は困難であり，通常の内視鏡下鉗子生検では病理組織学的診断も困難であることが多い．超音波内視鏡下穿刺吸引法（endoscopic ultrasound-guided fine needle aspiration；EUS-FNA）は，SMTを安全かつ正確に病理組織学的に診断できる非常に有用な検査法である．画像のみでは診断が困難な10mm以上の低エコー充実性腫瘍はEUS-FNAの適応を考慮すべきである．

[ Summary ] Gastrointestinal submucosal tumors (SMTs) are often incidentally detected during gastrointestinal endoscopy. SMTs are difficult to diagnose using conventional endoscopic images and forceps biopsies, because SMTs are covered with normal epithelium. Endoscopic ultrasound-guided fine needle aspiration (EUS-FNA) is used for safe and accurate histological examination of SMTs. The indication of EUS-FNA for SMT is the presence of a hypoechoic solid tumor larger than 10 mm, which cannot be diagnosed conclusively by imaging alone.
