【 要旨 】 十二指腸にみられるSMT様隆起のほとんどは良性であり，ブルンネル腺過形成，嚢胞やリンパ管腫などである．一方，治療対象となる病変は，NET，GIST，十二指腸粘膜下層以深への浸潤癌，悪性リンパ腫，転移性腫瘍，gangliocytic paragangliomaであり，その疾患頻度はまれである．十二指腸SMT様病変の，白色光内視鏡における良悪性の鑑別には，色調・形態やサイズに加え，鉗子による感触でcushion signの有無が重要である．超音波内視鏡で，主座やエコーレベルにより，鑑別診断がある程度可能であるが，cushion sign陰性のSMTでは表面の陥凹やびらん，潰瘍からの生検が望ましい．

[ Summary ] Most of the submucosal tumors (SMTs) frequently observed in the duodenum are benign lesions such as Brunner's gland hyperplasia, cyst, and lymphangioma. Duodenal tumors such as neuroendocrine tumor, gastrointestinal stromal tumor, invasive carcinoma, lymphoma, metastatic tumor, and gangliocytic paraganglioma are rare but are indications for treatment. Color, shape, size, and consistency through touching with forceps are important factors in the differential diagnosis of duodenal SMTs based on white-light endoscopy. Endosonography is useful for further assessment of origin and internal echo, but biopsy from surface depression, erosion, or ulcer is recommended for SMTs with a negative cushion sign.
