【 要旨 】 |
乳頭部を除く表在性十二指腸腫瘍は,従来まれな腫瘍といわれていたが,近年になり内視鏡機器の開発,内視鏡医の技術の進歩および認知度の上昇などによりその報告は徐々に増えてきている.腺腫や粘膜内癌に対しては低侵襲治療が求められているが,その臓器特異性および治療難易度の高さから大型の十二指腸腫瘍に対する内視鏡的粘膜下層剥離術(ESD)は一般化できていないというのが現状かと思われる.われわれの部門では十二指腸ESDの際の工夫としてwater pressure methodおよびstring clip suturing methodを治療および術後偶発症対策として行っている.今後の課題としては治療症例の集約と保険の見直しが必要かと考える |

[ Summary ] |
A superficial duodenal tumor is a rare condition. Recently, with advances in endoscopic techniques and improved training and experience gained by endoscopists, the detection rate of duodenal tumors has increased. Minimally invasive treatment such as endoscopic resection is needed for adenomas and mucosal adenocarcinomas; however it is difficult to perform endoscopic submucosal dissection (ESD) for large lesions secondary to technical difficulties and organ specificity. We use the water pressure method that creates a mucosal pocket and the string clip suturing method to avoid delayed complications. Centralization of duodenal ESD cases and re-evaluation of insurance are issues that need to be managed in this context. |
